Birth & Death Certificates
General information: Certified copies of Missouri birth and death certificates are provided by the Vital Records department Monday-
A Notary Public is available to notarize affidavit for corrections, paternity affidavit, and out of state birth and death certificates.

Certified copies of birth certificates for persons born in Missouri from 1920 to present can be obtained in our office for $15.00 per copy. Cash, check, money order or credit card (processing fees apply) are all accepted forms of payment.

Request for Birth Certificate Application to Jefferson City
If a birth occurred before 1920, you can click on the link above to request an application from vital records in Jefferson City.
Certified copies of death certificates are available for deaths occurring from 1980 to present. Death Certificates are $13.00 for the first copy and each additional copy is $10 each issued to the same person in the same visit. Cash, check, money order or credit card (processing fees apply) are all accepted forms of payment.

Request for Death Certificate Application by Mail
Applications requested by mail MUST BE NOTORIZED before the death certificate can be issued. Applications not notarized will be returned. Please do not send cash in the mail.
NOTE: The Howell County Health Department can only print Death Certificated for years 1980 and up. Please use the application to Jefferson City for all other years.

Certificates for births, deaths, marriages and divorces that took place outside of the state of Missouri can be obtained by visiting the National Center for Health Statistics website.
